Cardio OnDemand

Advancing Cardiovascular Care Through Innovative Solutions

Cardio OnDemand™

CIS is leading the region in technological advancements. Cardio OnDemand acts as a virtual front door for CIS patients by providing three unique programs: CardioConnect call center, Telecardiology, and Cardio@Home Patient Monitoring. These programs, available to our Louisiana patients, feature state-of-the-art technology, increasing access to care, improving outcomes, and enhancing communication between the patient and the physician.

CardioConnect™

For your convenience, the CardioConnect call center is available to our Louisiana patients Monday-Friday from 7 a.m. until midnight and provides additional support for the entire CIS team. CardioConnect staffs nurse practitioners and nurses to answer questions and concerns regarding your care as quickly as possible.

The CardioConnect team works seamlessly with each patient's physician to deliver quality care.

Louisiana patients may continue to call their local CIS office with questions, but may receive a return phone call from the CardioConnect call center, which is based in Lafayette, Louisiana.

Telecardiology

CIS offers telecardiology services to remotely-located hospitals and physician practices, allowing patients to receive world-class cardiology care without leaving their community. CIS telecardiologists provide consultations in the emergency room and on the department floor, improving access to care for our patients.

Facilities offering CIS telecardiology services include:

  • Lady of the Sea General Hospital in Galliano, Louisiana
  • Abrom Kaplan Memorial Hospital in Kaplan, Louisiana
  • Dr. Brian Higgins' Family Practice in Brusly, Louisiana

Additionally, CIS telecardiology services will soon be available at Acadia General Hospital in Crowley and St. Martin Hospital in Breaux Bridge.

Cardio@Home™ Patient Monitoring

Cardio@Home utilizes state-of-the-art technology to monitor patients, providing consistent, continual care when needed. The Cardio@Home care team features physicians, nurses, and hospital case management employees that all work together toward the common goal of improving patients’ health and well-being. Family members are also integrated into the care plan to provide extra support.

This remote patient monitoring program reduces risk of readmissions and improves quality of life for patients, who can have peace of mind knowing that their care is consistently being monitored when leaving a hospital.
