Leading the region in technological advancements, CIS's Virtual Care Center enhances care options for our patients by providing unique programs such as the CIS OnDemand call center, Telecardiology, Cardio@Home remote patient monitoring and CardioConnect, a mobile app and patient portal. These programs feature state-of-the-art technology, increasing access to care, improving outcomes, and enhancing communication between the patient and the provider.
For your convenience, the CIS OnDemand team is available to our Louisiana patients 24 hours a day, 7 days a week and provides additional support for the entire CIS team. CIS OnDemand staffs nurse practitioners and nurses to answer questions and concerns regarding your care as quickly as possible.
Our team works seamlessly with each patient's physician to deliver quality care.
Louisiana patients may continue to call their local CIS office with questions, but may receive a return phone call from the CIS OnDemand Call Center, which is based in Lafayette, Louisiana.
CIS offers telecardiology services to remotely-located hospitals and physician practices, allowing patients to receive world-class cardiology care without leaving their community. CIS telecardiologists provide consultations in the emergency room and on the department floor, improving access to care for our patients.

Cardio@Home utilizes state-of-the-art technology to remotely monitor patients at home, providing consistent, continual care when needed. The Cardio@Home care team features physicians, nurses, and hospital case management employees that all work together toward the common goal of improving patients’ health and well-being. Family members are also integrated into the care plan to provide extra support. This remote patient monitoring program reduces risk of readmissions and improves quality of life for patients, who can have peace of mind knowing that their care is consistently being monitored when leaving a hospital.
CIS CardioConnect Patient Portal and Mobile App allow patients convenient, secure access to their health record, including lab results and the ability to request medication refills and appointments. To sign up, ask a receptionist for your special 8-digit passkey at your next visit. Click here to access the patient portal. The CIS CardioConnect mobile app can be downloaded from the Apple App Store. It is currently only available on Apple mobile devices.
OMS C3: Chronic Care Management
Another new, critical service that has been added to the Virtual Care Center is our Chronic Care Management Program: OMS C3. OMS C3 allows a CIS provider to connect with patients via non-face to face visits such as phone calls, emails, text messages, and video to help patient monitor their chronic conditions. Chronic Care Manager is a way to increase the interactions with our patients to improve their overall care. In addition, more frequent patient/provider interactions increases the patient’s quality of life, lowers mortality, and lowers hospital readmissions.
