Understanding Paravalvular Leaks: How One Procedure “Flipped a Switch” for Fred

After a quadruple bypass 12 years ago, Fred Cavin was told his heart would likely be good for about 10 years. Exactly 10 years later, he suffered a heart attack and underwent a stent placement and valve replacement. However, after the procedure, Fred knew something still wasn’t right. Despite being told to return for a routine follow-up, he trusted his instincts and continued seeking answers.

After being referred to CIS by his primary care doctor, Dr. Phillip Smith, interventional cardiologist at CIS in Baton Rouge and Zachary who specializes in treating structural heart and valve disease, discovered that Fred had a significant paravalvular leak (PVL) – a rare complication in which blood leaks around the outside of a replacement valve. The leak was causing Fred’s worsening symptoms, including severe fatigue, shortness of breath, and a decline in his quality of life; even preparing food left him drained. Through a minimally invasive procedure, Dr. Smith placed a small plug to seal the leak and restore normal blood flow. 

Fred noticed a dramatic improvement within hours of the procedure, describing it as feeling like someone “flipped a switch.” Today, he is grateful he trusted his instincts, found a care team that listened to his concerns, and received specialized cardiovascular care close to home. 

Dr. Smith emphasizes that early evaluation can help prevent serious complications. Many heart conditions can be diagnosed with noninvasive testing, such as ultrasound imaging, making it easier to assess symptoms without immediately resorting to invasive testing. “If you feel like something is wrong, listen to your gut and get evaluated,” he said.
